I think that there are relationships between the people around batman who support him as a superhero that are richer for having seen the first movie, but in terms of plot there's not any serious problem with not having seen the first one. If you know the common knowledge of batman's life you should be okay.
Gordon, for example, is only a Lt., not a Commissioner. And Rachel is the character who was his childhood sweetheart and was part of the first film. I don't know much about Batman comics mythology so I don't know how close the movies are to traditional canon.
But if you know Bruce Wayne is a billionare playboy who is a secret nighttime vigilante... that should be enough.
